Parents, you'll need to fill out all the forms for each of your children.
- Please read the Notice of Privacy Practices PDF.
- Then download, print out, and complete the other three (3) patient forms:
- Patient Acknowledgement and Consent
- Patient Information
- Adult Dental Care Survey or Child Dental Care Survey
- Bring the completed forms with you to your appointment.
